I acknowledge that I am aware of Saddleback Unified School District Policy (Board Policy 5131.6) on alcohol, drugs or narcotics during Grad Night. In addition, I waive any claims against the venue, the District, El Toro High School, the ETHS PSTO, all PSTO officers, employees & agents, volunteers, chaperones, or security for injury, accident, illness or death, or lost items occurring during or by any reason of Grad Night.
I also acknowledge that Parents/Guardians of students not following rules will be called to pick up their student immediately or within a reasonable time of incident. The student will not be able to re-enter the party or be refunded any portion of the ticket price. Complete rules available on our website www.ethspsto.org
By attending, I grant irrevocable and unrestricted right of PSTO to use digital photographs/video of my student for publicity or any other purpose and in any manner/medium, including website and internet promotion. I waive all claims and liability to said digital images/video.
I agree that my student will ride the bus TO and FROM Grad Night activities. Any deviation from this must be approved by Parent/Guardian and Grad Night Chair PRIOR to this event. Students may NOT leave during the event
I acknowledge that a parent/guardian will pick-up my student at approximately 3:00am (final time TBD) at ETHS following the event. Students are not to drive themselves home after being up all night. PSTO is not responsible for students after they return from Grad Night.
In addition, I am aware of the Education Code Section 1081.5 which provides that all persons making a field trip or an excursion are deemed to have waived all claims against the venue, SVUSD, ETHS, the ETHS PSTO, all PSTO officers, employees & agents, parent volunteers and committee chaperones for injury, accidental illness, or death occurring during or by reason of the trip or excursion.
Should my student be injured, the PSTO/School Administration has my permission to secure on-the-spot medical treatment. Parent/Guardian will be contacted as soon as possible for direction and notification and I will be responsible for payment of all medical services rendered.
I have received and understand the conditions of the off-campus activity and give my consent for my student to participate.

Every year there are students that cannot afford to attend Grad Nite. We would love to be able to help as many students as possible attend Grad Nite with full or partial scholarships. Scholarships are vetted by ETHS Counselors and kept confidential. Donations are tax deductible. Thank you in advance for your generosity!
Grad Nite is a PSTO-organized event and NOT included in an ASB package. All Grad Nite fees go toward the costs of the event, transportation and any other Grad Nite expenses.
